Diagnostic Team

For an FASD diagnostic team to exist the members need a coordination and referral hub. FASDx Services LLC provides this service for the team. The office reviews and accepts the referrals, collects intake information and relevant records, and then selects team members who will see the client.

Coordination and Referral Staff

Most often a client will see four health professionals- a medical provider, psychologist or neuropsychologist, speech language pathologist, and occupational therapist. They each evaluate the client in their own office and send a copy of their report back to our office. Payment is expected at the time of service in these offices.

All of the team members accept Medicaid and private insurance. The entire team is trained in using the University of Washington FAS 4 Digit Code process as required by the State of Alaska FASD Provider Agreement that funds the office. Team members work under a Memorandum of Agreement with FASDx Services LLC.
